Since 2009, the Milbank Memorial Fund’s Multi-State Collaborative has brought together diverse groups of professionals from public and private payer organizations, state agencies, health care providers, and non-profits representing projects across the country actively engaged in multi-payer primary care transformation. Our aim is to support these local teams of payers, project facilitators, and others as they help their local multi-payer primary care transformation tables stabilize and succeed, leading to sustainable and credible local leadership and successful project outcomes.
