Primary Care Transformation

Despite being the only health sector shown to improve population health and health equity, primary care in the United States is in crisis as access to care declines. The Milbank Memorial Fund supports policymakers and other decision makers in strengthening the field of primary care.

Good primary care is essential to improving population health. The Milbank Memorial Fund approaches strengthening primary care by focusing on 1) increasing primary care investment, 2) improving primary care access and delivery with payment reform, and 3) tracking the health of US primary care.

Primary Care Investment Network

Convened by the Primary Care Development Corporation and supported by the Commonwealth Fund and the Milbank Memorial Fund, this network comprises advocates and officials from more than 20 states who are actively pursuing policy to increase spending on primary care at the state level.

Health of US Primary Care Scorecard

The Milbank Memorial Fund and The Physicians Foundation, in partnership with the Robert Graham Center, publish an annual “Health of US Primary Care Scorecard” to measure the implementation of high-quality primary care and inform national and state policy.
