FOREWORD
This compilation of reports from leaders of 62 community health partnerships is a publication of the New York State Community Health Partnership (NYSCHP). Its purpose is to encourage sharing of information and ideas among leaders from different communities.In the spring of 1998 the New York State Department of Health, a founding member of NYSCHP, invited leaders from across the state to nominate or report on projects in preparation for a conference on Communities Working Together for a Healthier New York. The Milbank Memorial Fund, an endowed foundation that has worked in public health since 1905, is publishing and distributing these reportsó62 that were completed of 73 nominations submittedóon behalf of NYSCHP, of which it is also a member.
The NYSCHP, founded in 1997, is a collaboration among leaders and organizations from the public and private sectors to improve the health and well-being of New Yorkís communities. Collectively and individually, members of the NYSCHP serve as catalysts and facilitators for health improvement activities throughout New York State.
The communities whose leaders carried out these projects and prepared these reports represent the diversity of New York State. These communities are located in every region of our state. They range in size from villages to large cities.
Each project defines and responds to the needs of a particular community. Categorized in retrospect, the projects address six overlapping clusters of issues: (1) access to care, primary care, and service integration; (2) community health improvement; (3) prevention and treatment of disease; (4) injury prevention; (5) healthy births, children, and youth; and, (6) mental health and substance abuse.
The reports offer examples, and there are others across the state, of how committed and skilled people address local needs and circumstances. The projects illustrate people working together to address priorities that they chose. They are not ìbest practicesî since what works in one community may not work the same way in another.
The Steering Committee of the NYSCHP stands ready to provide support and information to leaders in local communities interested in improving community health.
